Gather Supporting Documentation
To support your work experience and training, gather the following documentation** to upload with your online application in MyCert.
Training Documentation
Acceptable documentation may include:
copies of training certificates
letter of completion
signed statement of completion of training from the trainer or training company executive
Experience Documentation
When submitting your experience, make sure documents clearly state whether the experience is in RAM or XR.
Acceptable documentation, verified by a current Radiation Safety Officer (RSO) or a company personnel officer, may include:
Past radiation employment records
Traceable documentation
Signed statement from employer
Additionally, submit a completed employment history form:
